East Liverpool City Schools is excited to welcome our newest Potters! 🎉 If your child will be 5 years old on or before September 30, 2025, it's time to begin the enrollment process.

✅ Start Online: Visit http://eastliverpool-oh.finalforms.com/ to complete all required forms.

📌 Prepare the following documents:
📄 Birth certificate (showing age eligibility)
💉 Immunization records (including Hepatitis B, DPT, Polio, MMR, and Varicella)
🏡 Proof of residency (utility bill, driver’s license, etc.)
⚖️ Custody paperwork (if applicable)
